Assisting in thoracentesis

Guide in assisting in thoracentesis:

1. Ascertain in advance if chest Roentgenograms and or other test has been prescribed or completed for accuracy and guides the physician the extent of injury of the lungs. This should be available at the bedside.

2. See if consent from has been explained and signed.

3. Determine if the client is allergic to the ideal anesthetic agent to be used. Give sedation if prescribed.

4. Explain the procedure to client and indicated how he can be helpful. Discuss the following:
         a. Nature of the procedure
         b. Importance of remaining immobile/ refraining from coughing.
         c. Pressure sensation to be experienced.
         d. No discomfort is anticipated after the procedure.
         e. Readjust gown if needed; put lining (draw sheet/ draw sheet).
         f. Ask permission from client to arrange for the procedure.



5. Clean bedside table and overbed table.

6. Go back to nurse' station and perform handwashing.

7. Prepare the equipment.

8. Bring the equipment to client's room.

9. Provide privacy.

10.Position the client: (upright, sitting on the sign of the bed or sitting on a chair).

11.Arrange equipment inside client's room.

12.Open client's room and expose the back.

13.Perform medical handwashing.

14.Prepare the site of puncture: (2nd and 3rd intercostals space midclavicular line).

15.Do an after care of the prep tray.

16.Perform medical handwashing.

17.Open the sterile pack (thoracenthesis set).

18.Put in sterile gloves.

19.Arrange the specimen bottles near the client.

20.Prepare the anesthesia.
    Surgeon drapes the client:
    administer anesthesia, nurses stays with patient and gives the following instruction.
 
      a. Deep breathing when anesthesia is administered when needle is inserted.

21.Assist in dressing (give plaster).

After care:

22.Place client on bed rest; record v/s 15 minutes for 1 hour; record the amount of fluid (color and viscosity). Administer oxygen.

23.Label specimen bottles.
     Evaluate the client.
